RESPONSIBILITIES:

  1. Manages scribe staff to correctly summarizes and documents what the physician says during an examination, including but not limited to: proper charting of the examination, documenting additions to the chief complaint, assessment and recommendations for treatment (including justification of decisions reached when appropriate).
  2. Meets weekly with clinical management team.
  3. Meets monthly with compliance officer.
  4. Observes the patient and the physician and assists when appropriate during the exam, including but not limited to: holding the patient’s head in the slit lamp, providing additional medications and/or instruments, following directions in a medical emergency.
  5. Maintains an awareness of the patient’s verbal and non-verbal communication when the physician is providing instructions as it may necessitate the scribe repeating information, providing written instructions, and/or patient education materials for the patient to review at home.
  6. Initial training for all scribes. Will develop and implement training programs (classes, manuals, and education).
  7. Periodic review of charts to assure standards for documentation and coding are met.

Assures that scribes adhere to the following:

  1. Maintains patient flow, by ensuring a minimum of two patient rooms are always occupied.
  2. Remains as close to the physician as possible and limits time away from the physician and the patient.
  3. When applicable, prepares accurate eye glass or contact lens prescriptions and/or medication prescriptions for the eye physician’s signature prior to the completion of the examination.
  4. Alerts eye physician to patient-related forms that need completion.
  5. Completes the superbill and/or the check-out form, providing direction to the patient prior to following the physician.
  6. Guides patient to front desk after completion of exam and ensures proper follow-up appointments are made. Learns the personality and preferences of each physician, recognizing differences and abiding by them when working with that particular physician.
  7. Becomes fluent in the exact requirements of each examination.
  8. Accommodates the needs of the patient for the exam:Monitors examination room and any physical limitations the patient may have.
  9. Makes sure the patient chair is down before the patient prepares to leave, the equipment is off, the slit lamp is out of the way, etc.
  10. Provides patient with cane, handbag, coat, etc. to expedite patient flow.
  11. Evaluates the condition of the examination room to ensure adequate supplies are in stock.
  12. Notifies physician of changes to the daily schedule, especially emergencies.
  13. Performs other tasks as required.

 

EDUCATION AND EXPERIENCE:

  1. Fluency in English, bilingual preferred.
  2. Two years experience in a medical office scribe.
  3. Excellent spelling ability.
  4. Ophthalmic coding certification, or obtained within 1 year.
  5. Certified ophthalmic assistant preferred.

Responsibilities
Essential duties include but are not limited to:

  • Interview patients and document ocular and medical histories into Electronic Medical Records system.
  • Perform a variety of routine tests using ophthalmic instruments that provide diagnostic information including but not limited to intraocular pressure, basic motility, visual fields, fundus photography OCT, refractions, A-scans, topography, and FAs.
  • Assist the physician during diagnostic and minor surgical procedures, sterile preparations, and documentation.
  • Administer topical medications or diagnostic drugs as directed by physician.
  • Maintain confidentially of patient records.

  • Perform basic triage.

Responsibilities:

Performs all activities that relate to reprocessing of reusable medical supplies, linen, equipment, and instrumentation including decontamination, cleaning, inspection, assembly, wrapping, sterilization and storage.   • Responsible for patient check-in, greeting everyone in a pleasant and professional manner.

Patient Check-Out

  • Check-out all patients. Review fee sheets for correct charges and diagnosis marked; enter information into computer.

  • Be sure the patient understands the doctor’s instructions.
  • Check that patients have been given appropriate prescriptions.
  • Schedule return appointments for follow-up, as necessary.
  • Schedule patients for transfer of care and any required diagnostic testing.
  • Explain all fees and patient financial responsibility.
  • Verify patient insurance and address information.
  • Secure all necessary patient signatures; obtain proper informed consent and insurance authorization.
